We participate with most major insurance companies including: United Healthcare, Anthem Blue Cross and Blue Shield, Arise Health Plan, Trilogy, HealthEOS, Network Health Plan, WEA Trust, Aetna, Wisconsin Medicaid, Galaxy Health Network, Molina Healthcare, ppoNext, and Pyramid Life â€“ Todayâ€™s Options. We also accept Medicare Assignment. We strongly encourage you to verify network status with your insurance prior to scheduling surgery.

Bellin Orthopedic Surgery Center will submit charges to your insurance. Most insurance plans process claims within 30 days. After receiving your insurance processing, we will issue you a statement with your balance.

Your insurance carrier will be billed according to the information provided on the day of your visit. You must provide the correct insurance information to the clinic and/or surgery center within 30 days of your appointment or insurance will not be billed. Please contact your insurance company prior to your visit to check your benefits and ensure we participate in your plan.
If your insurance carrier does not pay, you are responsible to contact them to resolve the matter.
After your insurance pays, any remaining balance such as deductibles, co-pays, self-pay or non-covered charges are billed to you and are your responsibility. Payment in full is due within 30 days of receiving your first statement.

To make a payment, you may choose any of the following options:
- Cash, Credit Card, Debit Card, or Check
- An interest free short term payment plan with BOSC for three months. (See Payment Plans)
- A long term payment plan via our financial partner, HELP Financial. (See Payment Plans)
Short Term Plan
This payment plan is not to exceed three months. There is no interest or fees associated with this plan. Three equal monthly payments would be made directly to Bellin Orthopedic Surgery Center.
Extended Payment Plan
This repayment plan is administered through HELP Financial, our financial partner. Terms vary with options ranging from 12, 24, and 36 months. The first 12 months are always at no cost to the patient and there are no prepayment penalties. There is a $10.00 annual fee if you exceed 12 months and an 8% annual interest rate that equals .67% a month. Interest is only calculated on the current balance for that month. There is a required minimum monthly payment.
HELP Financial is a Third Party Administrator for our Extended Payment Plans. Therefore, due to the healthcare privacy laws, we are required to have you sign the â€œLoanâ€ agreement that outlines the terms of the plan and gives written authorization for your financial information to be exchanged between both entities.
Although they refer to the contract as a loan, HELP Financial is not a bank or credit card company. There are no credit checks or reporting to credit bureaus through HELP Financial.
