PAYMENTS & INSURANCE
Bellin Orthopedic Surgery Center will submit charges to your insurance. Most insurance plans process claims within 30 days. After your insurance has processed your claim, we will issue you a statement with your balance.

If your insurance plan is not listed, please call us at 920-430-8120 to verify if your plan is in-network.
Your insurance provider will be billed according to the information provided on the day of your visit. You must provide the correct insurance information to the clinic and/or surgery center within 30 days of your appointment or insurance will not be billed. Please contact your insurance company prior to your visit to check your benefits and ensure we participate in your plan.
If your insurance carrier does not pay, you are responsible to contact them to resolve the matter.
After your insurance pays, any remaining balance such as deductibles, co-pays, self-pay or non-covered charges are billed to you and are your responsibility. Payment in full is due within 30 days of receiving your first statement.
